Ferritin is the cheap blood test most men never order
Why iron can be low or quietly high in men after 35, what ferritin actually measures, and when a 'tired' story needs a lab, not another stack.
Ferritin is the cheap blood test most men never order
Men will buy a sleep ring, a wolf-logo hormone tub, and a membership they do not use. They will not ask for ferritin. Ferritin is a storage protein. It is a cheap line on a blood form. In men it can be low, which is miserable, or quietly high, which is how hereditary haemochromatosis sits in a family for a decade while everyone talks about “getting older.”
I am not diagnosing you. I am saying tiredness, joint aches, and a bronze-ish tan you did not earn are not always a motivation problem.
What ferritin is, without the brochure
Ferritin reflects stored iron. It is not the same as haemoglobin. You can have a normal full blood count and a ferritin that is a story. Inflammation can also lift ferritin, so a single high number is not a tattoo. Context matters: CRP, symptoms, family history, alcohol, supplements.
Low ferritin in a man is less common than in menstruating women, but it happens: blood donation, gut loss, poor intake, endurance work that you treat as a personality. High ferritin in a man is not rare enough to ignore, especially if relatives had “liver issues” nobody named.
Hereditary haemochromatosis is a genetic iron-loading condition. It is treatable. It is also easy to miss if nobody orders the cheap test.
Why the supplement aisle makes this worse
Men take multivitamins with iron “because more is more.” Many adult men do not need extra iron. Extra iron is not a vitality potion. It is a mineral that stores.
If you already eat red meat most days and you swallow an iron tablet because a gym advert said “oxygen,” you may be adding to a tank that is already full. That is the opposite of optimisation.
I stopped assuming “low energy” meant “need minerals.” Sometimes it means sleep. Sometimes it means depression. Sometimes it means the tank is full and the liver is not amused.
A draw I delayed for no good reason
I was 41, donating blood on a schedule I liked, and also taking a “performance” multi because it was on offer. I felt flat. I blamed screens. Ferritin later was not dramatic in either direction, but the exercise of asking for it ended the guessing. The multi with iron went in the bin. Donation timing got a conversation.
That is the useful version. Not a miracle. A closed loop.
If ferritin is clearly high, you do not start chelating yourself from a forum. You get a clinician, maybe transferrin saturation, maybe HFE genetics, maybe a conversation about venesection. If it is clearly low, you find the leak before you play chemist.
What I write on the form
Ferritin. Full blood count. If the story is tiredness: TSH, maybe B12, morning testosterone if symptoms match, HbA1c if the family tree says so. I do not order a 40-marker “longevity panel” as a first move. I order the cheap things that change decisions.
Fasting is not always required for ferritin. Follow the lab sheet. Do not lift heavy an hour before and then argue with inflammation.
How this page fits the rest of the log
I treat ferritin like resting heart rate: boring, repeatable, easy to skip. The write-up I send is ferritin, the blood test most men never order. It is blunt about high and low, and it does not sell a detox.
If your clinic shrugs at a request for ferritin, ask once more and then ask another clinic. A cheap storage marker is not an exotic ask.
Things that are not ferritin
A hair mineral test from a shopping centre. A “heavy metals” Instagram story. A sauna as chelation. Those are entertainment.
Coffee with meals can nudge absorption. That is a diet detail, not a protocol. Alcohol can lift ferritin through the liver. That is a reason to be honest on the form.
If you donate, tell the person reading the result. Donation is a feature, not a bug, until it is the reason you are made of paper.
A small rule
I do not add iron unless a lab and a person with a licence said so. I do not ignore a high ferritin because I “eat clean.” I do not treat a normal haemoglobin as proof the stores are fine.
Tired men buy stacks. Slightly less tired men buy one blood test and then buy less. I like the second group better.
Family history people treat as folklore
“Uncle had a liver.” “Dad was always tired and bronze in winter.” “We are a Celtic family and we joke about rust.” Those jokes are sometimes HFE. I am not a genetic counsellor. I am a person who thinks a ferritin request is cheaper than another decade of folklore.
If a relative had venesections and nobody told you why, ask. Then ask for your own number.
Women in the family who menstruate can look different on the same gene. Do not use a sister’s “normal energy” as your clearance.
Donation, steak, and the story you tell
Regular donation is a public good and a ferritin variable. Say it on the form. A stack of steaks is also a variable. Say it if you eat like a cartoon.
Endurance blocks that surprise you with flatness deserve a look at stores, not only at “need more beetroot.” If you are already high, more red meat is not a pep talk.
I do not fear meat. I fear guessing.
What I do after the result, in principle
Low, with a leak to find: clinician, not a cart of iron gummies.
High, with a family story: clinician, saturation, maybe genetics. Not a sauna cleanse.
Normal, still tired: sleep, mood, thyroid, the boring list. Ferritin did its job. It closed a door.
Closing doors is the whole point of cheap tests. The aisle wants doors open forever.
